The shape
Process passive: werden in the right tense + past participle. Das Formular wird ausgefüllt. Das Formular wurde gestern ausgefüllt. Das Formular ist ausgefüllt worden. The last one is the Perfekt, and the worden (not geworden) is the detail people forget.
von, durch, and leaving the agent out
von plus a person: Der Brief wurde von der Chefin geschrieben. durch plus a thing or means: Die Stadt wurde durch ein Hochwasser zerstört. Very often there is no agent. That is the point of the passive: the form, the rule, the result — not the hero.
sein + participle is not the same machine
Die Tür ist geöffnet can describe a state (the door is in an open condition). Die Tür wird geöffnet describes an event. Mixing them is how technical texts start to mean the wrong time. If you mean a process, keep werden.
When not to force it
German is happy with man: Man spricht hier Deutsch. That is often more natural than Deutsch wird hier gesprochen, especially in speech. Use the passive when the patient of the action is the topic — reports, rules, news — not to look advanced.
